TheCareClinicprovidesaninvaluableservicetothecommunityofSummitCountyanditsneighboringcommunities.Ilovetheprogressivenatureoftheirapproachandthefactthattheytreatthewholeperson.Theimpactstheyhaveontheirpatientsarenotshortterm.Theyarechangingthelivesofourresidentsnotonlyinanemergencybutforthelongterm.WeallbenetbecauseoftheCareClinic.RobMillisorDeveloperBreckenridgeGrandVacationsThemissionofSummitCommunityCareClinicistoprovideexceptionalintegratedpatient-centeredhealthcareservicesdesignedtomeettheneedsofpeoplewhoexperiencebarrierstoaccessingcareregardlessoftheirabilitytopay. fourpageRecentAccomplishmentsImprovedPayerMixOverthecourseof2014theCareClinicworkedaggressivelytoimproveanddiversifyitspayermixtoincreasesustainabilityanddecreaserelianceonoutsidefundingsources.Throughenhancedoutreacheffortstoenrollpatientsinpubicandprivateinsuranceandimprovedbillingandcollectionprocedurespatientservicesgenerated26oftheCareClinicsrevenues1047795in2014.Thisrepresentsa21increasefromjustthreeyearsago.Seethepayermixchartonpageeleven.PursuitoftheFederallyQualiedHealthCenterDesignationTheCareClinicisaFed-erallyQualiedHealthCenterFQHCLook-AlikefacilitybutisworkingtowardfullFQHCdesignationtoaccessincreasedbenetsforimprovedclientservice.Duringatwo-daysitevisitinMaytheHealthResourcesandServicesAdministrationHRSAreviewedeveryaspectoftheCareClinicsadministrationandoperationandinearlyOctobertheCareClinicsubmitteditsFQHCapplication.TheCareClinicwilllearnaboutapotentialFQHCdesignationinMay.IncreasedDonorGivingBetween2013and2014theCareClinicincreasedindividualgiftsby22resultingina65000increaseinannualgiving.EverygiftnomatterwhatsizemakesanimpactfortheCareClinicanddemonstratesahighlevelofcommunitysupport.InadditiontoincreasedannualgivingtheCareClinicalsoreceivedaspecialseriesoflargedonationsin2014.Thespeciallargegiftswereinitiatedbyanofferfromananonymousdonortomatchdollarfordollaruptothreeindividualgiftsof50000.Threemajorsupporterstookthechallengeresultinginacombined300000giftfortheCareClinic.ThemajorsupportorswereRobandAmyMillisorMikeMillisorandMaryJoSokolowskiandRickandSusieGrossman. vepageRecentAccomplishmentsPatientPortalInApriltheCareCliniclauncheditspatientportalprovidingpatientstheabilitytoaccesshealthinformationviaasecureweb-basedsystem.Patientscanrequestpersriptionrellsreceivelabresultsandaskquestionsoftheirprimarycareproviderviathisservice.APortalEnrollmentSpecialisthiredoverthesummerisavailableintheCareClinicwaitingroomtohelpfacilitatepatientenrollmentinthesystem.SincestartingtheprograminAugusttheSpecialisthasenrolled841patientsintheportal.TheCareClinichopestohaveatleast50ofpatientsattheCareClinicenrolledintheportalbytheendofthenextscalyear.ExpandedQualityAssuranceEffortsInOctober2013theCareClinicwasawardedagrantthroughtheOfceofRuralHealthPolicytohireaQualityAssuranceManagerandDataCoordinator.TheQualityAssuranceManagerworkswithteamsthroughouttheCareClinictoanalyzeoperationsfromasystemslevelidentifyareasforimprovementandtestpossiblesolutions.Thisprocesshasledtoanewcultureofdata-drivendecisionmakingbyCareClinicmanagement.ExamplesofprojectsthathavebeencompletedthisyearincludeimprovedpatientaccesstoappointmentsimprovedpatientexperienceandCareCliniccustomerserviceincreaseinHypertensivepatientswhohavetheirbloodpressureincontrolandadvancementsintheeligibilityandregistrationprocess.
